What to Bring to Your Appointment
- Your insurance card
- Information pertaining to your medical and surgical history
- An updated list entailing your current prescriptions including dosages and over-
the-counter medications you are currently taking
- Bring any x-rays, test results or medical records you may have
At your first appointment, our office staff will have you complete several patient information forms. For your convenience, the forms are accessible from our website (see below). You will need Adobe Acrobat Reader to download the forms. If you don't have the program, you can download it free of charge, by clicking here: Adobe Acrobat Reader. To view and then download or print forms, simply click on the highlighted words.

Prescriptions and Refills
It is best to call for prescriptions and refills between 9 a.m. and 4 p.m., weekdays.
This allows us to review your chart for allergies and accuracy. If an emergency occurs after hours make sure to have your medication name, dosage and the number of your pharmacy.
We can only prescribe medications to patients who are actively in our care.

Insurances Accepted
Our physicians are providers for several insurance plans. Prior to your appointment
each patient should check their insurance manual to insure their physician is a
participating provider. Each patient should review their policy to be aware of
rules and limitations for their plan.

Patients with urgent problems can usually be seen the same day.
If you cannot make your appointment, please call to cancel at least 24 hours prior to your scheduled time so other patients can be accommodated.
